We are recruiting for a dedicated and supportive Patient Support Officer to enable people of all ages living with sight loss to make informed choices about their future.

Duties will be to provide Information, Advice and Guidance to people living with sight loss and provide solution focused interventions to enable an individual to cope with the challenges faced due to their sight loss.  As part of the First Steps service you will provide people living with sight loss the opportunity to access Henshaws Pathway to Independence and other local services to help people to live independently in their community.

Responsibilities will include:

  • To provide impartial information, advice and guidance to patients and their families.
  • To provide person centred solutions to enable individuals to overcome the emotional, physiological and practical challenges faced following a diagnosis of an eye condition.
  • To share knowledge and information with health care professionals situated at our Altrincham hub.

Understanding of the needs of clients with visual impairments and/or other disabilities, along with the ability to provide a comprehensive  assessment of an individual’s needs, is essential. The successful candidate will need to have proven experience in providing people with Information, Advice and Guidance have a background in rehabilitation, nursing or enablement.
